Notice: Under Missouri statutes, sales tax refunds belong to the retailer who collected, accounted for, and remitted those taxes. This form allows High Profile to voluntarily share proceeds of its refund with qualifying customers. Customers will receive High Roller loyalty points equivalent to the amount of tax they paid. Customers must be members of the High Roller loyalty program to receive their points.
